Anytime Health, Dental and Vision Plan Changes Available Now!

Effective July 1st 2012, families can now request plan changes at anytime during your child’s enrollment in Healthy Families.

Previously, you could request a plan transfer for your child under certain circumstances.

Contact Healthy Families by phone, mail or fax to request a plan transfer.

• Call us at 1-866-848-9166 to tell us your new plan choice. (The call is free.)
• Or, write your plan choice and your Family Member Number on a piece of paper and mail the paper to:

  P.O. Box 138005
  Sacramento, CA 95813-8005

• Or, fax the paper to: 1-866-848-4974 (The fax number is free.)

Healthy Families will send you a letter to let you know when you can begin using the new plan. This plan change may take up to 40 days to be effective.
