New Applications and Forms on Health-e-App

Health-e-App is now available in English for completing the Annual Eligibility Review (AER) and Add a Person (AAP) applications, Program Review and Continued Enrollment forms. These applications and forms will soon be available in Spanish.

Since December 2010, Californians have been able to apply for Healthy Families using the Health-e-App (HeA) online application with or without the help of Certified Application Assistants (CAAs). Now, a family or CAA can complete an AER or AAP application online! You can also request a review of a HFP decision with the Program Review and the Continued Enrollment forms.

To access the new online applications and forms a family must have a Family Member Number (FMN) and a HeA account. To connect to your Healthy Families account from the Health-e-App website, you must request a Personal Identification Number (PIN).
