What if I Want to Change Plans?
You can change your plans anytime from the date your child is enrolled. To find out what plans are available to you, click here.
If you want to change plans, call 1-866-848-9166. You can call between 8 a.m. and 8 p.m., Monday through Friday, and Saturday from 8 a.m. to 5 p.m. The call is free! Or, send your written request to:

Plan transfers will begin on the first day of a month following the transfer request, if the request is received on or before the tenth (10th) of the month. If the request is received after the tenth (10th) of the month, the effective date of the transfer is the first day of the second month following the transfer request.
Note: Starting November 1, 2009, dental plan choices for some families who do not have any child enrolled in Healthy Families for two (2) consecutive years will be limited to certain dental plans. Two (2) consecutive years means that your child is (or was previously) enrolled in Healthy Families for 2 years without a break in coverage.

WHEN WILL HEALTHY FAMILIES ASK YOU TO CHOOSE A NEW PLAN?
If Healthy Families learns that your child no longer lives in an area served by your chosen health, dental, or vision plans, Healthy Families will notify you in writing to choose new plans.
If you do not choose new plans within 30 days from the date of the written notice, Healthy Families will enroll your child in the Community Provider Plan in your new area, if one is available. Healthy Families will choose the dental or vision plan for your child if there is more than one dental or vision plan in your child’s area.
